Obtaining the Form:

  1. Start a claim: You can initiate a claim by calling the “PIP new claims” phone line provided by the Department for Work and Pensions (DWP). After the call, you’ll receive the PIP2 form, along with an information booklet, in the mail.
  2. Alternatively: If phone calls are not convenient, you can send a letter requesting the form to “Personal Independence Payment New Claims.” You’ll then be sent the relevant paperwork.

Completing the Form:

  • The PIP2 form is quite detailed, so allow yourself ample time to go through it thoroughly.
  • The information booklet accompanying the form provides guidance on completing each section.
  • When answering questions, be honest and provide as much detail as possible about how your disability impacts your daily activities.
  • You have one month to complete and return the form after receiving it.

You can start a claim by post instead, but it takes longer to get a decision.

Send a letter to ‘Personal Independence Payment New Claims’.
